Required Forms for start of each school year

Annual Required Health Forms - this explains the health forms that are required before the start of school every school year.

Upload to FACTS Family Portal

• Universal Health Form (Physician Signature Required) - required for all students

• Virgin Islands Immunization Registry Form (VIIR Form) - required for all students

• Yellow VI Immunization Record - use your existing yellow card and upload it to FACTS Family Portal

• Food Allergy and Anaphylaxis Care Plan (Physician Signature Required) - required for students with food allergies requiring Epipen

• Prescription Medication Authorization Form (Physician Signature Required) - required for students with any medication to be dispensed at school

• Pre-Participation Physical Evaluation (Physician Signature Required) - Sports Physical for Interscholastic Athletics Association - required for all athletes elementary and up

• Sports Waiver - required for all athletes Elementary and up

• DOE School Bus Application and VIMSIA Bus Waiver - required for bus riders only
